This information will allow us to be more productive with our time, as it covers basic information we would otherwise need to obtain from you, such as your personal information, names of family members, assets, and liabilities. If you are not sure about exact figures or informationbut (or if the information requested is not readily available), your best estimate is sufficient for our initial consultation. Please fill out as much as you can; we can always collect this information from you later if necessary.

Please also be sure to read the Terms of Engagement. The Terms and Conditions set forth the scope of our representation, the obligations we have to each other for disclosure and confidentiality, dual representation issues for married clients, our policies relating to retention of your documents (if you choose to leave them with us), our duties if you become disabled, and other important information.
